Can Zepbound cause depression?

Probably not, but it’s still a good idea to monitor your mood when starting any new medication. 

Recent anecdotal reports of mental health issues on GLP-1s (aka glucagon-like peptide-1 receptor agonist drugs) have raised concern about Zepbound and depression, but there’s no evidence to suggest a direct link. Zepbound’s drug label doesn’t even list depression as a side effect, and the warning for suicidal thoughts and behaviors was removed after the United States Food and Drug Administration (FDA) requested its removal from all GLP-1 drug labeling.

Tirzepatide, the active ingredient in Zepbound, is a dual agonist medication, with combined receptor agonists for GLP-1, along with GIP (glucose-dependent insulinotropic polypeptide). There are a handful of anecdotal reports about depression symptoms from people taking GLP-1s like semaglutide (Ozempic). The FDA is continuing to research the issue to evaluate the potential risk of depression on Ozempic, Zepbound, or other similar medications, but has requested that suicide warnings be removed from these drugs’ labels.

What research says about Zepbound and depression

Can Zepbound cause depression? Let’s unpack the research we have so far.

  • The big picture: Reports of depression made up just 0.6% of all side effects linked to GLP-1s in one study. Of the 187 total reports of depression, only four were from tirzepatide users. 

  • Ongoing research: The FDA’s Adverse Events Reporting System (FAERS) receives more than 2 million reports each year. Between 2004 and 2023, there were 262 reports of psychiatric symptoms — such as nervousness, stress, or disordered eating — on tirzepatide. But simply experiencing nervousness or stress while on tirzepatide does not prove that tirzepatide caused those effects. 

  • Clinical trial data: An analysis of clinical trials found that people taking tirzepatide had better depression ratings than those taking a placebo. This suggests that tirzepatide doesn’t increase the risk of depression.

  • Potential link to depression improvement: There is a report of two cases of people on tirzepatide citing an improvement in depression symptoms. Again, this does not mean tirzepatide caused the improvement in mental health (and two cases don’t tell us much), but it’s further evidence against Zepbound triggering depression.

So, can someone on Zepbound have symptoms of depression? Yes, but there’s simply no proof that the medication is to blame. 

Why some people experience depression on Zepbound

As we’ve seen, there’s no direct evidence that Zepbound causes depression — in other words, if you’ve heard the term “Zepbound depression” thrown around, just know it’s not a real thing. But if you’re experiencing depression symptoms while taking Zepbound, there are a few possible explanations.

Temporary blood sugar fluctuations

It’s possible that blood sugar fluctuations could explain some moodiness on tirzepatide. We know from animal studies that unstable blood sugar levels can also destabilize the mood. But that same research also shows that GLP-1s help regulate blood sugar, actually reducing irritability and anxiety-related behaviors. 

Tirzepatide helps regulate blood sugar, and at least theoretically, changes in blood sugar as your body adjusts to tirzepatide might temporarily affect your mood. Further research is needed to confirm this.

Emotional impact of weight loss

Some people lose significant weight on Zepbound. And significant weight loss can be an emotional ordeal, even when it’s something you want to happen. Research has found that depression is quite common after bariatric surgery, too. 

It’s not inevitable that losing weight will lead to depression. But it is important to remember that rapid changes to your body, self-perception, and even eating habits can feel stressful, and stress can contribute to depression

Unrealistic expectations

On the flip side, some people might feel sad or depressed if their tirzepatide experience doesn’t match their expectations. If you’re feeling down about how little your body has changed since starting this weight loss medication, you’re not alone. There are several possible explanations for not losing weight on tirzepatide

Talk to your prescriber to troubleshoot your specific situation. Your healthcare provider can offer suggestions for dose adjustments or lifestyle changes to support your weight loss goals. 

Physical discomfort

Gastrointestinal side effects are commonwith tirzepatide. Though side effects are typically temporary, they can still be frustrating and exhausting. Feeling sick and tired for weeks on end could contribute to sadness and a loss of enjoyment — symptoms that also describe depression

Bottom line: There isn’t a strong link between Zepbound and depression, but physical changes and discomfort, along with the stress of adjusting to a new body and lifestyle, could have an effect on your mood.

How to manage depression on Zepbound

If you notice that you’re feeling more depressed while on Zepbound, tell your healthcare provider. In the meantime, here are some general strategies to manage your symptoms:

  • Write it down: Start tracking your mood and what else is happening when you notice feeling sad, hopeless, or withdrawn. Noticing patterns and potential triggers can help you and your healthcare provider make a plan for moving forward.

  • Take care of your body: Research shows that practical self-care routines, like getting enough sleep, staying hydrated, and moving your body every day, can reduce symptoms of depression.

  • Identify your support network: However you’re feeling, you’re not the only one who has felt this way. Family, friends, a support group, and even your healthcare provider can offer important emotional support throughout your weight loss journey. 

  • Tell your prescriber: Zepbound itself has not been linked to depression, but it’s important to tell your healthcare provider what you’re experiencing. They can help pinpoint the cause of your depression and determine the best path forward.

Whether it’s Zepbound depression or not, any mood changes affecting your daily life shouldn’t be ignored. Pay attention, track what you’re feeling, and seek help so you can start feeling better as soon as possible.
