Yes, belly fat can contribute to erectile dysfunction. Visceral fat, the deep fat stored around your organs, is the main culprit.

One rough way to gauge whether you're carrying enough visceral fat to matter: a waist circumference above 40 inches (102 cm) is generally considered a marker of increased cardiometabolic risk in men.

It's thought to release hormones and inflammatory chemicals into your bloodstream that may interfere with your body's ability to get and maintain an erection.

Belly fat rarely works in isolation, though. It tends to trigger a chain reaction, disrupting your hormones, blood vessels, and metabolic health all at once. It’s why obesity and ED often go hand in hand.

How does belly fat affect testosterone?

Belly fat and testosterone have a complicated and two-way relationship.

Excess visceral fat increases the activity of an enzyme called aromatase, which converts testosterone into estrogen. The more belly fat you carry, the more testosterone gets converted, leaving you with less of the hormone your body needs for a healthy sex drive and strong erections. Lower testosterone, in turn, promotes even more belly fat, which creates a self-reinforcing cycle. 

And it's not just your sex life that can take a hit. Lower testosterone can sap your energy, affect your mood, and chip away at your confidence.

Some research suggests men carrying excess weight may be more likely to develop ED compared to men at a healthy weight, and hormonal imbalance is a big part of why.

How does belly fat affect blood flow and erections?

An erection depends on blood flowing into the penis quickly and staying there.

Belly fat can interfere with that by damaging the inner lining of your blood vessels, making it harder for them to relax and open properly. The result is less blood reaching the penis, which can mean weaker erections, or even trouble achieving an erection in the first place. 

The arteries in the penis are smaller than those elsewhere in the body, sothey often show the effects of poor vascular health first. Although addressing belly fat is a long game, treating the ED itself doesn't have to be.

PDE5 inhibitors, such as Viagra (sildenafil) andCialis (tadalafil), can help by relaxing blood vessels and improving blood flow to the penis, which can help many men get an erection, whatever the underlying cause.

They won't undo the underlying hormonal or vascular effects of visceral fat. But they can make it easier to get and keep an erection while you work on the root cause.

In addition to the brand-name and generic options mentioned, there are other non-pill formulations that feature certain PDE5 inhibitors. Ro, for example, also offers:

Can losing weight improve ED?

Yes, losing weight, particularly belly fat, can make a real difference to your erections.

Men with obesity and ED who cut their calorie intake and got more active lost an average of around 33 pounds over two years. Plus, more than 30% got their erectile function back entirely, whereas just 5% of men who made no lifestyle changes saw improvements.

You don't need to hit a specific number on the scale to notice a change. Even losing 5%–10% of your body weight has been shown to help, at least in men with obesity and type 2 diabetes.

Losing weight consistently improves erectile function in men carrying excess weight. Diet-induced weight loss improved sexual function in men with obesity, both with and without diabetes.

It's not a guaranteed fix, and results vary. But if belly fat is driving your ED, addressing it directly gives your body a real shot at recovery.

How to reduce belly fat

There are a few consistent habits that can go further than any single fix for reducing belly fat. None of these needs to be perfect to make a difference; consistency is more important than precision.

Move most days

Aiming for about 150 minutes of moderate aerobic activity a week can help reduce waist circumference and body fat. Aerobic exercise has also been shown to improve blood vessel function and erectile function in people with overweight or obesity.

Eat more like the Mediterranean

In a study of more than 21,000 men, greater adherence to a Mediterranean-style diet (think: lots of vegetables, fruits, nuts, legumes, and fish) was associated with a lower risk of developing ED over time. (This effect was strongest in men under the age of 60, though all ages did experience a benefit.) 

Cut back on alcohol and prioritize sleep

Heavier drinking is linked to more visceral fat, and chronic alcohol use appears to suppress testosterone production as well. 

Losing a full night's sleep or more has also been shown to lower testosterone, which can negatively impact sex drive and contribute to ED. (It can also play a role in weight gain or impede weight loss.) Point being: Both booze and bedtime are worth paying attention to alongside diet and exercise.

What about rapid weight loss and erectile dysfunction?

Losing weight steadily is one thing, but crash dieting is another. Cutting dietary fat too low can actually lower testosterone levels in the short term, which works against your sexual health rather than helping it.

Regular aerobic exercise, such as walking, cycling, or swimming, can also help support erectile function by keeping your blood vessels healthy.

Since losing belly fat can improve erectile function, weight loss from any approach may help over time. If you're using a GLP-1 medication, such as the Wegovy pen or Wegovy pill (semaglutide), Zepbound (tirzepatide), or Foundayo (orforglipron), to lose weight, talk to your healthcare provider about how it may affect your sexual health specifically. After all, research on GLP-1s and erectile function is still developing.
