Does Aetna cover Zepbound?
Aetna’s coverage of Zepbound can vary by plan, as each plan can determine which GLP-1s (if any) are covered and to what extent.
In mid-2025, Aetna announced that some plans would no longer cover Zepbound. But it said it will continue to cover other GLP-1s, such as Wegovy (semaglutide) and Saxenda (liraglutide), as well as certain weight loss pills like Qsymia (phentermine/topiramate) and orlistat.
Indeed, many Aetna plans still list Zepbound as an excluded drug in their formularies for 2026. A formulary includes the list of drugs covered by your plan. These plans include:
Instead of Zepbound, these plans list orlistat, Qsymia, Saxenda, and Wegovy as preferred options. However, the plans also note that it’s possible for a non-covered drug (e.g. Zepbound) to be covered if the preferred option is not clinically inappropriate.
So, it may be possible for your healthcare provider to request Zepbound coverage on your behalf. But securing coverage may be a long shot.
It’s worth noting that drug formularies can change at any time; Aetna updates them every quarter. In other words, Aetna may cover Zepbound again in the future. For example, starting April 2026, the Aetna Basic Control with ACSF plan will drop brand-name Saxenda in favor of generic Saxenda, aka liraglutide, and Zepbound.
Does Aetna cover Zepbound for weight loss?
At the time of publication, only one Aetna plan covers Zepbound for weight loss: Aetna Basic Control with ACSF.
Other than that, many Aetna plans do not cover Zepbound for weight loss, including Aetna Advanced Control and Aetna Standard Control plans. Instead, these plans cover orlistat, Qsymia, Saxenda, and Wegovy.
Does Aetna cover Zepbound for obstructive sleep apnea?
As of publication, a handful of Aetna plans, including some Aetna Better Health Medicaid plans, cover Zepbound for obstructive sleep apnea (OSA). That’s largely because the injection is the only FDA-approved medication for OSA at the moment.
In general, though, Aetna plans vary in their coverage of Zepbound for OSA.
Do you need prior authorization for Zepbound?
Similar to other insurance plans (e.g. Cigna), when Aetna covers Zepbound, it typically requires a prior authorization.
This means your healthcare provider will need to submit paperwork to Aetna showing that you meet specific coverage criteria for Zepbound. These requirements can vary depending on whether you’re taking Zepbound for weight loss or sleep apnea.
Prior authorization for Zepbound for weight loss
If your Aetna plan covers Zepbound for weight loss, your prior authorization paperwork must include documentation that shows that you have:
Participated in a comprehensive weight management program that included lifestyle changes, a reduced-calorie diet, and increased physical activity for at least six months prior to trying weight loss medication
A body mass index (BMI) of ≥30 or a BMI of ≥27 and at least one weight-related health condition (e.g. high cholesterol, type 2 diabetes, high blood pressure)
If your Aetna plan does not cover Zepbound, your prior authorization paperwork must show that you are an adult who:
Can’t use any of your plan’s preferred (i.e. covered) weight loss drugs because you’re allergic to their ingredients, have a contraindication that makes them unsafe for you, or you’ve already tried them and they didn’t work or caused too many side effects
Has a BMI of ≥40
Prior authorization for Zepbound for sleep apnea
To get Aetna to cover Zepbound for sleep apnea, your healthcare provider must provide documentation that you are an adult with a:
Diagnosis of moderate to severe OSA with an apnea-hypopnea index (AHI) of at least 15 events per hour on either a polysomnogram or home sleep apnea test
Current BMI ≥40

How to check if your Aetna plan covers Zepbound
To find out if Aetna covers Zepbound, you can review your plan’s online drug formulary, contact your plan directly, or use Ro’s (free!) GLP-1 Insurance Coverage Checker.
To review your plan’s online drug formulary:
1. Visit the Aetna member website. Log in to access the drug formulary for your plan, which will list all the drugs currently covered by your plan.
2. If you don’t have your login information handy, scroll down and select your plan year and plan name. Your plan name should appear.
3. Click on your plan name. On the following screen, click “Find Medicine.”
4. Enter “Zepbound” and press the “Search” button.
5. From the list that appears, select your prescribed dosage of Zepbound. The following screen will show you the coverage status.
You can also check your coverage by calling Aetna directly using the number on the back of your insurance card. Once you reach a representative, ask if your Aetna plan covers Zepbound. If they say “yes,” ask for the expected copay or coinsurance and the specific steps required for you to receive coverage (e.g. submitting prior authorization).

How much does Zepbound cost?
Without insurance, Zepbound can cost anywhere from $299 to over $1,000 per month. How much you pay depends on the pharmacy, the dose prescribed, and whether you’re using Zepbound pens or vials.
If you pay cash for your prescription, you can get Zepbound for much less on Ro. Specifically, Zepbound vials are available (if prescribed) on Ro for the following cash prices:
$299 per month for the 2.5 mg dose
$399 per month for the 5 mg dose
$449 per month for the 7.5 mg, 10 mg, 12.5 mg, and 15 mg doses
If you fill your prescription at your local pharmacy and you don’t have insurance coverage for Zepbound, your price may be at or around the list price of $1,086.37 per fill. But if you use Lilly’s KwikPen Self-Pay Savings Card, you can fill your Zepbound prescription at your local pharmacy starting at $299 per month.
What to do if Aetna won’t cover Zepbound
If your Aetna plan doesn’t cover Zepbound, you still have options. For example, you can:
You can appeal Aetna’s decision. If Aetna rejects your coverage request, they’ll send you a letter telling you why and provide instructions on how to appeal their decision. You have 60 days from the date of this letter to appeal the coverage denial.
Or you can ask about Zepbound alternatives. Call Aetna, review your plan’s formulary, or use Ro’s insurance checker to see if your plan covers Mounjaro (which has the same active ingredient as Zepbound but is approved for type 2 diabetes), other weight loss GLP-1s like Saxenda or Wegovy, or prescription weight loss pills.
If any are covered, talk to your healthcare provider about potentially switching treatments and if doing so would help your chances of coverage. (For example, taking Mounjaro off-label can also pose a challenge if you don’t meet the necessary requirements.)
How to save on Zepbound
There are several ways to save on Zepbound if your Aetna plan doesn’t cover it.
Consider cash-pay options. Thanks to Ro’s partnership with Eli Lilly, you can get Zepbound pens and vials for $299–$1,050 per month.
Try Zepbound coupons. You can download pharmacy-specific coupons for free through sites like GoodRx and SingleCare. Visit the website, select your dosage and pharmacy, and save or print the card to show your pharmacist.
Get a longer supply of Zepbound. Sometimes, filling a two- or three-month Rx can be more cost-effective than a one-month supply.
