With a list price of over $1,300 per month, the cost of taking Wegovy (semaglutide) can add up very quickly, especially if your insurance does not cover the injectable weight loss medication. 

Wegovy may be prescribed to people with obesity or overweight and at least one weight-related health condition, such as type 2 diabetes or high cholesterol. In 2024, the US Food and Drug Administration (FDA) approved Wegovy for another indication besides weight loss: to reduce the risk of heart attack, stroke, or death in people with obesity or overweight and established heart disease. 

Whether you are taking Wegovy to lose weight or lower your cardiovascular risk (or both), there are options to lower the cost of this medication. Read on as we break down how the program works and share other ways to get Wegovy for less.

How much does Wegovy cost? 

The list price of Wegovy is $1,349.02 for a 28-day supply, which includes four injection pens prefilled with a single dose of the medication (Wegovy is administered weekly). The list price of a medication describes the price set by the drug manufacturer and does not factor in any discounts, rebates, or insurance coverage. Your out-of-pocket cost for Wegovy will depend on your insurance coverage and the pharmacy you use. 

How do you get $25 Wegovy?

Getting Wegovy for $25 is a great deal—particularly considering that the list price of Wegovy comes out to $16,188.24 per year. But there are actually ways for some people to get Wegovy entirely free. Currently, you can get Wegovy for as little as $0 for up to thirteen 28-day fills of Wegovy (just over one year of treatment). Then, after the 13th refill, you may pay as little as $25 for a monthly supply of Wegovy, which includes four one-time-use injection pens. According to Novo Nordisk, this offer is effective as of January 2, 2024.

However, here’s the fine print: to get Wegovy for $25 or as little as $0, you need private or employer-sponsored insurance with coverage for Wegovy and you must apply for the Wegovy savings card through Novo Nordisk. If your plan does not include coverage for Wegovy, you can still save up to $500 on a 28-day supply using the Wegovy savings card. If you don’t have health insurance, you won’t be able to get Wegovy for $25 or for free. The offer is also not available to people who have prescription drug coverage through a state or federal program such as Medicaid, Medicare, Medigap, VA, DOD, or Tricare. However, if you’re an uninsured cash payer through the Wegovy savings card, you might be able to reduce your costs by up to $500 a month. 

Other ways to lower the cost of Wegovy

If you’re not eligible for the Wegovy savings card, you may have other options for lowering the cost of Wegovy, depending on your insurance plan:

  • Private insurance or insurance through an employer: If your insurance plan offers coverage for Wegovy, you may qualify for the Wegovy savings card mentioned above. If it doesn’t, your healthcare provider may be able to request coverage on your behalf. Novo Nordisk offers a sample letter they can use here.

  • Medicare: In general, Medicare does not cover medicines for obesity management, but some Medigap and Medicare Advantage plans do. However, now that the FDA has approved Wegovy for an additional indication (lowering cardiovascular risk in adults with heart disease and obesity or overweight), coverage may expand for Wegovy. According to a 2024 memo issued by the Centers for Medicare & Medicaid Services (CMS), reducing cardiovascular risk is considered a medically-accepted indication, so individuals taking Wegovy for this reason may have coverage through Medicare Part D.

  • Medicaid: Medicaid drug coverage can vary by state. Similar to Medicare, drugs that treat obesity may not be covered. However, with the new FDA-approved indication for Wegovy as a treatment for reducing cardiovascular risk, coverage for Wegovy may expand. Your healthcare provider may need to submit prior authorization on your behalf.

  • Veterans Administration: If you have prescription drug coverage through the VA, you may be able to get Wegovy covered under the MOVE! Weight Management Program. Talk to your local provider and consult the drug formulary to understand your options.

  • Tricare: Wegovy may be covered under Tricare. Your healthcare provider may need to submit a coverage review. Talk to your local provider to understand your options.

In addition to working with your healthcare provider and insurance plan, you may be able to save more on Wegovy with a prescription discount card. These cards work as coupons at local pharmacies and can offer significant savings on prescription drugs.
