In early 2026, Eli Lilly announced a new form of Zepbound (tirzepatide). The weight loss medication now also comes in a multi-dose KwikPen with lower cash-pay pricing. So, how does the Zepbound KwikPen work, how much does it cost, and how can you get it?

What is the Zepbound KwikPen?

The Zepbound KwikPen is a prefilled, multi-dose injection pen. It contains four weekly doses of the tirzepatide drug in a single delivery device. 

To inject Zepbound using the KwikPen, you turn the dose knob to prime your pen, attach a small needle, select your dose, and push down. Those are the steps at a high level, anyway (we go into more detail below). 

The Zepbound KwikPen can be prescribed for:

  • Weight loss and management in adults with obesity or with overweight and a weight-related health condition (e.g. diabetes)

The multi-dose KwikPen can be a good option for people looking for something that’s more compact for travel and storage, and produces less waste than the single-dose pens or vials. However, it’s worth noting that it’s only available to people who pay cash for Zepbound and don’t have government-funded insurance, such as Medicare.

How many doses come in the Zepbound KwikPen?

Each Zepbound KwikPen contains four doses (a month’s supply of treatment) of the same dosage strength. The KwikPen is available in all six dosage strengths of Zepbound.

How much does the Zepbound KwikPen cost?

The Zepbound KwikPen costs $299–$499* for a one-month (28-day) supply, depending on dose. Lower doses of Zepbound cost less per month than higher doses. 

Here’s how much the Zepbound KwikPen costs when you pay cash on Ro (if prescribed): 

  • $299 per month: 2.5 mg dose

Who is eligible for the Zepbound KwikPen?

The Zepbound KwikPen is only available to people paying cash for their prescription on Ro. It can also be purchased for cash through LillyDirect or another one of Eli Lilly’s authorized telehealth partners. It’s not available to those who want to pay for their Rx using federal or commercial health insurance. 

If you have commercial health insurance, but it doesn’t include coverage for Zepbound, you’re still eligible to get the KwikPen. And in that case, it may be more affordable than buying the Zepbound single-dose pens at your preferred pharmacy. 

How to get the Zepbound KwikPen 

You can only get the Zepbound KwikPen with a prescription from a licensed healthcare provider, which is then filled through Ro, another authorized telehealth partner, or LillyDirect. 

Here’s how to get the Zepbound KwikPen (or another weight loss medication) on Ro:  

1. Online visit. Answer a handful of questions about your health history and weight loss goals to be shared with Ro. 

2. Clinician evaluation. Within a few days, a Ro-affiliated provider will review your answers and determine if you’re a fit for Zepbound (or another GLP-1). 

3. Prescription and delivery. If you’re eligible and interested in paying cash, your provider will then write you a prescription for Zepbound KwikPens. Ro will fill your Rx and then ship it directly to your door. (It should arrive in under a week!) 

4. Treatment and ongoing care. As a Ro Body member, you’ll have regular check-ins with your provider and receive access to your care team 24/7. Other perks include app-based features like treatment notifications, side effect guidance and FAQs, educational materials, and more. 

Keep in mind that the cost of the Zepbound KwikPen (or any other cash-pay GLP-1) is in addition to the price of the Ro Body membership. The program starts at just $45 for the first month and then $145 per month thereafter. 

How to use the Zepbound KwikPen

While your Zepbound KwikPen will come with specific instructions that you should review and follow, here are the general steps for using the injector: 

1. Get ready for your injection. Wash your hands with soap and water. Gather your supplies: KwikPen, needle, alcohol swab, gauze or cotton ball, and sharps disposal container.

2. Inspect the KwikPen label and liquid medication. Pull the KwikPen cap straight off. Confirm that the label shows your correct dosage strength and that the expiration date hasn’t passed.  The medication should be clear and colorless (or slightly yellow). If all looks good, use an alcohol swab to wipe the red inner seal on the KwikPen.

3. Attach the KwikPen needle. Select a new KwikPen needle and remove the paper tab from the outer needle shield. With the needle cap still on, push it straight onto the KwikPen and twist until tight. Remove the outer needle shield and set it aside. Remove the inner needle shield and discard it in your household trash.

4. Prime the KwikPen. Slowly turn the dose knob until you hear two clicks and see an extended line in the window. With the needle pointing up, hold the KwikPen and gently tap the liquid cartridge to collect any air bubbles. Push the dose knob in until it stops and hold it there while you count slowly to five. 

5. Confirm you primed it correctly. If the “0” icon appears in the window and you see a drop of liquid at the tip of the needle, move on to Step 6. Otherwise, repeat Step 4.

6. Choose an injection site. It can be your upper thigh, stomach, or the back of your upper arm. You’ll rotate injection sites each week.

7. Inject Zepbound. Turn the dose knob all the way until it stops, and you see the “1” icon in the window. Insert the needle into your skin at a 90-degree angle. Push the dose knob in until it stops and hold it for a slow count of five. Once you see the “0” icon in the window, pull the needle out of your skin.

8. Dispose of used needles and prep pen for storage. Carefully replace the outer needle shield, unscrew the capped needle, and throw it away in a sharps container. Then, put the KwikPen cap back on the pen and place it in its carton. You don’t need to return an opened KwikPen to the refrigerator, because it’s stable for 30 days at room temperature. Unopened KwikPens should be stored in the refrigerator. 

Zepbound KwikPen storage

When not in use, the KwikPen should be stored in its original carton. Specific storage instructions for the Zepbound KwikPen depend on whether you’ve already used it.

  • Before first use: Unopened Zepbound KwikPens should be kept in the refrigerator at 36°F–46°F (2°C–8°C). When stored in the refrigerator, the KwikPen is considered safe to use as long as it hasn’t expired. If needed, the KwikPen can be stored at room temperature up to 86°F (30°C) for up to 30 days.

  • After first use: Opened Zepbound KwikPens can be stored in the refrigerator at 36°F–46°F or at room temperature up to 86°F. 

Important: If a Zepbound KwikPen has been stored at room temperature for more than 30 days, it should be thrown away  — even if medication remains. Note that the used pen needles should be disposed of in a sharps container. The KwikPen itself can be disposed of in either a sharps container or the household trash.
