Does Humana cover Zepbound for weight loss?

Until recently, Medicare did not cover Zepbound or any other medications for weight loss. However, as of July 1, 2026, Humana and other Medicare plans cover the Zepbound KwikPen for weight loss through the Medicare GLP-1 Bridge. This is a temporary program that ends December 31, 2027. To be eligible, you must have Medicare Part D coverage through:

  • A standalone Medicare Part D prescription drug plan (PDP)

  • A Medicare Advantage coordinated care plan with drug coverage (MA-PD)

  • A Special Needs Plan (SNP)

  • An employer/union group waiver plan (EGWP0)

  • The Limited Income Newly Eligible Transition (LI NET) program

Does Humana cover Zepbound for sleep apnea? 

Yes, if you are prescribed Zepbound to treat moderate to severe obstructive sleep apnea (OSA), your Humana Medicare Part D plan may cover it. In this case, you would get Zepbound coverage through your Part D plan rather than through the Medicare GLP-1 Bridge, which is only for weight management.

It's typically covered as a Tier 4 medication and requires prior authorization. Tier 4 includes injectable GLP-1s and other more expensive medications. These medications have higher co-pays than generic or lower-tier drugs.

Do you need prior authorization for Zepbound?

Yes, Humana requires prior authorization for Zepbound, similar to other insurance plans like UnitedHealthcare and Aetna.   • You have a BMI of 27 or higher and prediabetes, peripheral artery disease, or a history of heart attack or stroke.

Prior authorization for Zepbound for sleep apnea

To get Humana to cover Zepbound for sleep apnea, your healthcare provider must submit prior authorization that confirms the following:

  • You have a diagnosis of moderate to severe obstructive sleep apnea (OSA).

  • Your BMI is 30 or higher, indicating obesity.

  • Your apnea hypopnea index (AHI) or respiratory event index (REI) is 15 or higher, based on the results of a polysomnogram or home sleep apnea test.

  • You will use Zepbound in combination with lifestyle changes that support weight loss, such as following a low-calorie diet and increasing your physical activity.

  • You do not have diabetes.

How to check if your Humana plan covers Zepbound

Your Humana formulary, called the Prescription Drug Guide, is a list of all the medications covered by your plan, along with their cost tiers and prior authorization requirements. Here's how to check if your Humana plan covers Zepbound:

  • Sign in to MyHumana and search the list of medications for “Zepbound.”

  • Print out the list of medications covered by your plan by visiting the Humana website, and selecting your plan type, year, state, and plan.

  • Call the Customer Care number on the back of your Humana member ID card.

How much does Zepbound cost?

The cost of Zepbound depends on your Humana plan.

  • Cost of Zepbound for weight loss: If your Zepbound prescription is covered under the Medicare GLP-1 Bridge program, you'll have a $50 monthly copay. Since the GLP-1 Bridge is a separate program from your regular Part D coverage, this Zepbound copay doesn't count toward your yearly deductible or out-of-pocket limit.

  • Cost of Zepbound for sleep apnea: If Humana covers your Zepbound prescription under Medicare Part D, your cost will depend on the details of your plan. For example, if you have type 2 diabetes, Humana may cover a GLP-1 like Mounjaro (tirzepatide), which contains the same active ingredient as Zepbound, or the Ozempic (semaglutide) injection or pill.
