Key takeaways

  • Most TRICARE plans cover Zepbound (tirzepatide), including TRICARE Prime, Select, and premium-based plans, but TRICARE For Life does not.

  • TRICARE requires prior authorization for Zepbound based on your body mass index (BMI), past weight loss efforts, and plan for lifestyle changes.

  • Zepbound can cost as little as $0 with TRICARE, depending on your military status and where you fill your prescription.

Does TRICARE cover Zepbound (tirzepatide)? Yes, most TRICARE plans cover Zepbound, but not all do, and TRICARE requires prior authorization first. TRICARE is the US military’s healthcare program for active duty and retired service members, National Guard and Reserve members, and eligible family members.

Does TRICARE cover Zepbound?

Yes, most TRICARE plans cover Zepbound with prior authorization if you meet certain clinical criteria and have a prescription from a TRICARE network provider. TRICARE covers Zepbound for weight loss if you have one of the following plans, including several overseas plans:

  • TRICARE Prime

  • TRICARE Prime Overseas

  • TRICARE Prime Remote

  • TRICARE Prime Remote Overseas

  • TRICARE Reserve Select

  • TRICARE Retired Reserve

  • TRICARE Select

  • TRICARE Select Overseas

  • TRICARE Young Adult

  • Continued Health Care Benefit Program

  • US Family Health Plan

While many TRICARE plans cover Zepbound, not all do. Notably, TRICARE For Life does not cover Zepbound. If you have TRICARE For Life, you may be able to get Zepbound KwikPens through the temporary Medicare GLP-1 Bridge by enrolling in Medicare Part D. The Bridge has a $50 monthly copay, covers only the KwikPen, and has its own body mass index (BMI) and health requirements. Approved prior authorizations are valid through December 31, 2027. TRICARE also does not cover Zepbound for people who are only eligible for care at military hospitals and clinics, or for certain other groups.

Like other insurers, such as Highmark and Cigna, TRICARE offers several health plans, and coverage may also depend on whether Zepbound is prescribed to lose weight or to treat sleep apnea.

TRICARE coverage for Zepbound: rules and criteria

Similar to other insurance plans that cover GLP-1s, such as Aetna and Blue Cross Blue Shield (BCBS), TRICARE requires prior authorization for Zepbound and covers it only for uses approved by the US Food and Drug Administration (FDA): weight management in adults with obesity, or with overweight and at least one weight-related condition, and moderate to severe obstructive sleep apnea (OSA) in adults with obesity. This means your healthcare provider must submit documentation showing that Zepbound is medically necessary for you before TRICARE approves coverage.

In August 2025, TRICARE announced changes to its coverage of weight loss medications, including Zepbound and Wegovy (semaglutide). The changes took effect August 31, 2025, and approved prior authorizations are no longer valid for beneficiaries with TRICARE For Life or direct care only.

Does TRICARE cover Zepbound for weight loss? Yes, if you meet certain criteria. For adults, your healthcare provider must show that you:

  • Have a BMI of 30 or higher, or 27 or higher with a weight-related health condition such as diabetes, OSA, osteoarthritis, metabolic dysfunction-associated steatohepatitis (MASH), or cardiovascular disease.

  • Have followed an eating plan and made lifestyle changes for at least six months without reaching your weight goals.

  • Have not lost at least 5% of your body weight after three months of generic weight loss pills such as phentermine, unless a medical reason prevents you from taking them.

  • Plan to continue making lifestyle changes, such as changes to your eating habits and exercise, while being treated with Zepbound.

Does TRICARE cover Zepbound for sleep apnea? Yes, but TRICARE has separate criteria for sleep apnea. For adults with moderate to severe OSA, your healthcare provider must show that you:

  • Are 18 or older with moderate to severe OSA, with an apnea-hypopnea index (AHI) of 15 or more events per hour

  • Have a BMI of 30 or higher

  • Have followed an eating plan and made lifestyle changes for at least six months without reaching your weight goals, and plan to continue them during treatment

TRICARE doesn’t approve Zepbound for any use if you’re pregnant, are taking another GLP-1 medication, or have a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia syndrome type 2. If you have TRICARE For Life, TRICARE doesn’t cover Zepbound for sleep apnea, but your Medicare Part D plan may cover it.

Do you need prior authorization for Zepbound on TRICARE?

Yes, TRICARE requires prior authorization for Zepbound.

Your healthcare provider must submit a form before TRICARE will cover Zepbound, and the prescriber must be a TRICARE network provider or a military hospital or clinic provider. This form must show that you meet TRICARE’s BMI requirements, have tried lifestyle changes, and will continue making lifestyle changes during treatment. If you take Zepbound for weight loss, the form must also show that you tried generic weight loss pills, unless a medical reason prevents you from taking them.

Once TRICARE approves your prior authorization, it lasts 12 months. To renew it, your healthcare provider must resubmit prior authorization each year and show that you have lost at least 5% of your starting body weight. If you take Zepbound for sleep apnea, renewal requires showing that your AHI has improved.

How to check if your TRICARE plan covers Zepbound

There are two ways to check if your TRICARE plan covers Zepbound:

1, Search your plan’s drug formulary. Visit the TRICARE Formulary Search on Express Scripts. In the “Medication” field, type in Zepbound, and select your sex and age. Then press Search to view your plan’s coverage for Zepbound. From there, click through to the plan and pharmacy type (military, home delivery, or retail) that applies.

2. Call TRICARE. You can find the phone number for your plan on the back of your TRICARE insurance card or on the TRICARE website. A TRICARE representative can check if your plan covers Zepbound and, if so, what the requirements are for getting coverage.

How much does Zepbound cost?

How much Zepbound costs with TRICARE depends on whether your plan covers Zepbound, your military status, and where you fill your prescription. If your TRICARE plan covers Zepbound, here’s what you’ll pay in 2026 based on your military status:

  • Other TRICARE beneficiaries, such as retirees and family members, pay $0 at a military pharmacy that carries Zepbound. Otherwise, it costs $44 per fill through home delivery or $48 per fill at a retail network pharmacy.

Without insurance, Zepbound costs $1,086.37 per 28-day fill at its list price, according to Eli Lilly, the manufacturer. On Ro, you can get Zepbound KwikPens at a lower cash price of $299–$449 per month, depending on the dose.*

What to do if TRICARE won’t cover Zepbound

If your TRICARE plan doesn’t cover Zepbound (or has stopped covering your medication), you still have options.

1. Pay cash for Zepbound. If TRICARE won’t cover Zepbound, you can pay the cash price for your prescription. On Ro, you can get Zepbound KwikPens for 299–449 per month, depending on the dose, if eligible and with a valid prescription.*

2. Use a pharmacy coupon for Zepbound. At a retail pharmacy, the cash price of Zepbound may be close to its $1,086.37 list price. However, you may be able to lower the price by using a pharmacy discount card from websites such as SingleCare and Optum Perks. Visit the website, search for Zepbound, and download the coupon for your preferred pharmacy. Eli Lilly’s Zepbound savings card isn’t available with government coverage, including TRICARE.

3. If you have type 2 diabetes, ask about other GLP-1s. Even if your TRICARE plan doesn’t cover Zepbound, such as TRICARE For Life, it may cover other GLP-1s when prescribed to treat type 2 diabetes. For example, TRICARE covers both Ozempic (semaglutide) and Mounjaro (tirzepatide, the same active ingredient as Zepbound) for type 2 diabetes, with prior authorization, but not for weight loss.

4. Consider other weight loss medications. If TRICARE denies Zepbound but your plan covers weight loss medications, you may qualify for another weight loss medication. TRICARE plans that cover Zepbound may also cover weight loss injections such as Wegovy (semaglutide) and Saxenda (liraglutide), or pills such as Qsymia (phentermine/topiramate), phentermine, and Contrave (naltrexone/bupropion). A healthcare provider can also discuss other oral options.

5. Change plans, if eligible. TRICARE Open Season runs Nov. 9–Dec. 8, 2026, and a Qualifying Life Event (QLE) allows enrollment changes within 90 days. Eligibility depends on the beneficiary category: Open Season doesn’t apply to active-duty members or TRICARE For Life beneficiaries, for example.

Bottom line

Getting Zepbound covered by insurance usually involves a few hurdles, and TRICARE is no different. If you have a plan that covers Zepbound and your healthcare provider submits prior authorization showing that you meet TRICARE’s coverage criteria, TRICARE may approve coverage. Here’s what to know about TRICARE and Zepbound:

  • Most TRICARE plans cover Zepbound, including TRICARE Prime, Select, and premium-based plans. TRICARE For Life does not cover Zepbound, but you may be able to get it through the temporary Medicare GLP-1 Bridge if you enroll in Medicare Part D.

  • TRICARE requires prior authorization for Zepbound. Your healthcare provider’s form must show that you meet the BMI requirements, have tried lifestyle changes, and plan to continue them. For weight loss, the form also asks about generic weight loss pills. Approval lasts 12 months, and renewal requires losing at least 5% of your body weight, or improving your AHI if you take Zepbound for sleep apnea.

  • You can check if your TRICARE plan covers Zepbound by calling TRICARE or searching your plan’s formulary online.

  • The cost of Zepbound with TRICARE can be as low as $0 for active duty service members and at military pharmacies. Without coverage, Zepbound has a list price of $1,086.37 per 28-day fill, and Ro’s cash prices are $299–$449 per month, depending on the dose.*

  • If TRICARE won’t cover Zepbound, you can pay cash for your prescription, use a pharmacy coupon card, or ask your healthcare provider whether you qualify for another weight loss medication or a GLP-1 for type 2 diabetes.
