Prior authorization for Zepbound for weight loss

Even if your plan covers Zepbound for weight loss, your policy may have strict eligibility criteria.

Generally, Highmark's coverage of Zepbound for weight loss is contingent on your healthcare provider confirming one of the following:​

  • You have a BMI of 27+ and a weight-related condition.

You can still fill a Zepbound prescription if your Highmark plan does not cover it. Here are some ways to lower the cost:
