Rybelsus vs. Wegovy: which is better for weight loss?

Wegovy is more effective for weight loss than Rybelsus. In clinical trials, nearly 90% of people taking Wegovy lost at least 5% of their body weight, compared with about 30% of people taking Rybelsus in a real-world study. 

It’s worth keeping in mind that Rybelsus is approved for type 2 diabetes, not weight loss (although it can be prescribed off-label for it). As a result, most studies of Rybelsus are conducted in people with type 2 diabetes, who tend to lose less weight on semaglutide than people without diabetes. However, even studies of people without diabetes show that Rybelsus produces less weight loss than Wegovy.

How much weight can you lose on Rybelsus vs. Wegovy?

People lose significantly more weight on Wegovy vs. Rybelsus:

  • Rybelsus: In clinical trials, people with type 2 diabetes lost 5–8 pounds in six months. In one small study, people with obesity lost 5.7% of their body weight — or 13 pounds — in one year.

  • Wegovy: In clinical trials of the Wegovy pill, people lost 13.6% of their body weight, or 31 pounds, after 64 weeks (about a year and three months). In clinical trials of the Wegovy injection, people lost 14.9% of their body weight, or 34 pounds, after 68 weeks (about a year and four months).

The Wegovy injection may be slightly more effective than the Wegovy pill for weight loss, but both produce significantly more weight loss than Rybelsus.

How long does it take to see results on Rybelsus vs. Wegovy?

People may begin to  see weight loss results as early as one month after starting Wegovy. In clinical studies, people lost about 2% of their body weight after one month on Wegovy. For Rybelsus, it typically takes longer — at least three months — to see a comparable amount of weight loss.

Rybelsus vs. Wegovy: doses and administration 

A major difference between Rybelsus and Wegovy is that Rybelsus is only available as an oral tablet, while Wegovy comes in both injectable and oral formulations. Both medications follow titrated dosing schedules, where you start at a low dose and gradually increase it over time. This allows your body to adjust to the medication while keeping side effects manageable.

Rybelsus

Rybelsus is an oral tablet that you take daily in the morning.

How to take it: Swallow the Rybelsus tablet whole in the morning on an empty stomach, with up to 4 ounces of water, at least 30 minutes before eating, drinking, or taking other medications.. 

Doses: Rybelsus comes in three strengths: 3 mg, 7 mg, and 14 mg.

Starting dose: 3 mg

Maintenance dose: 7 mg or 14 mg

Maximum dose: 14 mg

Dosing: After one month on the 3 mg dose, your healthcare provider will increase your dose to 7 mg. If you need more blood sugar control after 30 days on the 7 mg dose, they may increase your dose to 14 mg. Otherwise, you’ll remain at the 7 mg dose.

Wegovy

Wegovy is available as an oral tablet that you take daily in the morning, or as a weekly injection that comes in single-use, prefilled injector pens.

How to take it: Inject Wegovy subcutaneously (under the skin) of your thigh, abdomen, or upper arm on the same day each week. Take the Wegovy pill daily in the morning on an empty stomach with up to 4 ounces of water, at least 30 minutes before eating, drinking, or taking other medications.

Doses: The Wegovy injection comes in five strengths: 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, and 2.4 mg. The Wegovy pill comes in four strengths: 1.5 mg, 4 mg, 9 mg, and 25 mg.

Wegovy’s list price is $1,349.02 per month, which means it can cost that much or more when you fill your prescription at the pharmacy without insurance.

Insurance coverage: As a weight loss medication, Wegovy is less likely to be covered by insurance than Rybelsus, which is prescribed for type 2 diabetes. When Wegovy is covered, it may fall under a higher cost tier and require prior authorization.

Medicare Part D coverage: Wegovy is not covered by Medicare Part D when prescribed only for weight loss. However, it can be covered when prescribed for heart disease.

  • Switching from oral Rybelsus or Wegovy to injectable Wegovy: On the day after your last pill, start taking the 1.7 mg or 2.4 mg dose of the Wegovy injection. Inject Wegovy weekly moving forward.
